Someone asked me about alternate input devices, the catalyst being a friend who types with a mouth stick but who has some use of their hand. They were thinking of a twiddler type device. However, considering the single pointer limitation, I suddenly recalled a program called Dasher.
It works a lot like an "attack of the flying letters" video game. It also adapts to the input based on word frequency and spelling. While surprisingly intuitive, it does tend to penalize people who cannot spell.
Anyway, I just wanted to point out this program (ouch, leaving pun) to anyone typing with limited input options. As a proof of concept, I wrote this entire post using the program. It gets easier as you go, but I may have hurt myself...
